Form is an indie rock band formed in Itasca, Illinois, in the summer of 2018. The trio consists of drummer Alec Tau, singer and lead guitarist Jonny DiSilvestro, and bassist Kelly D. Their debut album, "E Fifth Ave," released in 2022, reflects the emotions and memories from their time together in Naperville, Illinois.
